RIVIAM has unlocked and automated the stream of patient discharge data between a hospital EPR and local third sector services.

The secure integration allows data to flow so acute teams using RIVIAM's Discharge service to discharge patients on pathways 0 and 1 can work seamlessly with housing, community and third sector organisations using RIVIAM to receive discharge referrals and co-ordinate care.

Removing delays in access to patient discharge information:

• enables better system-wide teamwork and decision making,
• utilises community and third sector capacity, and
• supports integrated care for patients, helping with their recovery and preventing re-admission.

Secure integration

In December 2023, RIVIAM enabled a data feed directly from the EPR Cerner Millennium at the Royal United Hospital Bath (RUH) to our Discharge service.

With the integration, patient data added or amended in the EPR flows seamlessly to update patient events in the Discharge dashboard.

This dashboard is used by acute ward teams at the RUH to see the status of referrals they have issued using an Onward Admission Checklist so they can fast track the discharge of medically fit patients.

The dashboard also provides the latest intelligence about a patient's situation to over 20 community, housing and third sector services using RIVIAM at the Community Wellbeing Hub in Bath and North East Somerset to receive the referrals, collaborate and co-ordinate services.
