With a RIVIAM enabled ‘Onward Admission Form’, ward staff discuss with patients about their needs and make referrals.

Multiple requests can be raised in one instance saving staff time finding different referral forms.

The checklist also presents the community services available increasing awareness of the types of support. People can request help from food packs to transport and help with urgent house repairs which might be preventing them returning home.

Referrals can be auto sent directly to a provider where they can be managed.

Referrals can be received and co-ordinated by multiple health, social care and third sector organisations using RIVIAM’s Multi-agency Referral Hub.
