A Discharge dashboard then provides clear indicators of the status of care being prepared or provided. This gives hospital teams confidence about community and third sector support arranged or delivered for people about to be discharged enabling quicker discharge decisions and freeing up beds.

Auto feed of hospital data

RIVIAM can take an auto feed of data from the hospital's EPR to update the Discharge dashboard with the latest information about a person's ward, Estimated Discharge Date and Actual Discharge Date.

This data is then securely shared with community and third sector providers using RIVIAM's Multi-agency Referral Hub and Discharge dashboard. This gives them greater confidence about when to visit a person and how best to co-ordinate care.
