Still managing multiple systems to deliver integrated care? Here’s what works. Our 2026 plans
26 January 2026

RIVIAM wishes you a healhty and joyful 2026
2.2 million eConsents. 50,000 community referrals. An AI Clutter Classifier.
What do they have in common? A single goal - making joined-up health and social care simpler, smarter and more accessible.
In 2025, we strengthened connections with customers across health, social care and the third sector - creating more integrated care solutions that improve people's lives.
We’re excited for 2026 as we continue to improve care co-ordination across partners, ensuring people receive the right care, at the right time, using the right information.

Integrated Neighbourhood Teams and community-based integrated care
We’re maintaining our focus on interoperability, alongside strengthening prevention and care co-ordination across local systems and partners. In 2026, we will:
- Enable Integrated Neighbourhood Teams with our Multi-agency Referral service for joined-up, community-based care across health, social care and voluntary services.
- Roll out our Advice & Guidance platform with NHS e-Referral Service (e-RS) integration, so clinicians can access specialist input and reduce unnecessary referrals.
- Launch the Referrer’s Hub, providing a secure space for referrers to log in, submit and track referrals and more easily communicate with delivery partners.
- Expand Sadie AI services as a core part of our care co-ordination solution, including summarising complex clinical reports to save time and improve decision-making.
- Introduce Digital Front Door and Single Point of Access (SPoA) Workflow services for HCRG Care Group’s Adult and Children’s Community Services across BaNES, Swindon and Wiltshire (BSW).
RIVIAM now integrates with Access Rio and EMIS Web
As a new member of The Access Group Partner Programme and the EMIS Partner Programme, RIVIAM integrates directly with the systems teams already use. Our existing real-time interface with TPP SystmOne™ automatically updates diaries and transactions every day.
No more siloed systems. No duplicate data entry. No switching platforms. Referrals are automatically created or updated to patient records in Access Rio (The Access Group) and we can search and retrieve a patient record, add consultation notes to the patient record, and create and manage appointments in EMIS Web (Optum), so information flows more seamlessly to enable timely patient care.

2025 impact

Immunisation achievements
In 2025, we expanded support for children and immunisation teams across all London boroughs and multiple new regions nationwide. Offering an end-to-end solution, we:
- Enhanced the Mobile Clinic App with features saving nurses time and improving accuracy, including updating consent status and changing vaccine type during clinics.
- Enabled immunisation teams to work more cohesively with a record number of school staff using our School Portal, helping manage clinics efficiently.
- Improved pre-vaccination checks by clearly flagging consents that require triage and highlighting ones for similar children, supporting safe and efficient clinics.

Joined-up community care
What did we achieve in 2025? Smoother referrals. Stronger multi-agency co-ordination. Solutions for more efficient operations and improved patient care.
- Two new referral portals/workflows went live (1 April 2025) for Mindworks Surrey (Surrey and Border Partnership NHS Foundation Trust) and Surrey Child and Family Health (HCRG Care Group), streamlining children’s physical and mental health community referrals.
- We improved access and triage through Forward Thinking Birmingham’s upgraded RIVIAM Referral Portal and SPoA Workflow (June 2025), capturing high-quality data for Eating Disorders and ADHD services.
- Our Patient Engagement Portal with Waitlist + Clinic Management and TPP SystmOne™ integration was launched, reducing Did Not Attends and increasing clinic efficiency for MSK services delivered by a national healthcare provider.
- Supporting NHS 10-Year Plan, we: enhanced multi-agency working (enabling Primary Care quick referrals, MDT Tasks and Dashboards) and completed phase one of NHS Wayfinder Programme to enable NHS App integration.
