World Health Day 2026: Why care co-ordination matters

On World Health Day, this year’s theme, “Together for health. Stand with science.”, reinforces how important co-ordination, collaboration and the right information are in connecting people to the right care service.

At RIVIAM, we support health and social care leaders with a secure care co-ordination platform and services that connect health, social care and VCSE. 

This helps Integrated Neighbourhood Teams work as a co-ordinated, integrated team rather than in isolation, improving health and wellbeing. By replacing paper referral forms, manual processes and disconnected systems, RIVIAM reduces costs, streamlines workflows and improves access, capacity and flow for more efficient, joined-up, personalised care. 

It reflects the direction of the NHS 10-Year Plan, with a stronger focus on community-based, digitally enabled and preventative care.

On World Health Day, we are proud to play our part in helping services work better together so people receive the right care, at the right time, with the right information.

Community Wellbeing Hub in Bath and North East Somerset (BaNES)
A multi-agency service bringing together over 35 health, social care and VCSE partners in BaNES to co-ordinate support more effectively in the community demonstrates how joined-up working can improve access to support and help people get to the right service sooner.

Forward Thinking Birmingham
A refreshed referral portal for children and young people’s mental health services across Birmingham, designed to make referrals easier and more effective. 

With a dynamic referral form, document postbox and enhanced pathways on RIVIAM, this supports more efficient referral management and care co-ordination in the Single Point of Access.

Mindworks Surrey
A secure referral service for children and young people’s emotional wellbeing, mental health and learning disability services in Surrey gives professionals and families a more streamlined way to request support, and helps services manage referrals smoothly and respond more effectively.

Surrey Child and Family Health
A dedicated referral service for children’s physical health services in Surrey, making it easier to refer into support such as physiotherapy, tongue tie and speech and language therapy.

Bath and North East Somerset, Swindon and Wiltshire (BSW) Community Services 
Our most recent partnership with HCRG Care Group went live on April 1st 2026

A digital front door for children and adult community services across Bath and North East Somerset, Swindon and Wiltshire, RIVIAM provides a simpler route into care, helping people and professionals refer to and access the right services more easily, supporting more joined-up community health delivery.
